At the head of Hanger family variation, the standard variation features the woman's legs an upright position, cradling the man's neck / head to keep balance. Not the most natural position to get into, we've found it easiest to start out with the woman lying on the edge of the bed to get the basic alignment, then rotating her upper body until she can reach down and plant her hands on the floor. If the height difference proves to be extra challenging, you may find it more enjoyable for the woman to plant her hands on an elevated surface, like an ottoman... just make sure it is very stable.
